CHW Services

a helping hand

We are excited to announce our partnership with CHAP to provide vital services to women not housed at Rubies. Our very own Alissa, a certified Community Health Worker, is here to assist women and pregnant individuals in accessing the resources they need within the community. She is dedicated to walking alongside you, offering support, guidance, and a helping hand through your pregnancy and life’s challenges. If you or someone you know could benefit from extra support, please fill out the CHW referral form or call 419.612.6399. Together, we can make a difference!

What exactly is a CHW?

A CHW stands for Community Health Worker. They are frontline public health workers who are trusted members of and/or have an unusually close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

What does a CHW do?

A Community Health Worker (CHW) performs a variety of roles, including:

1. Health Education: They educate individuals and communities about healthy behaviors and self-care techniques.

2. Outreach: They conduct outreach activities to engage the community and connect them with healthcare services.

3. Advocacy: They advocate for the health needs of the community, helping to address social determinants of health.

4. Support: They provide support to individuals, helping them navigate the healthcare system.

5. Referral: They refer individuals to appropriate health and social services.

Overall, CHWs play a crucial role in improving community health outcomes by bridging gaps between healthcare providers and the community.
